   The form itself. Currently at Rev. 4; the revisions are filed as Annex C.

2. SN-02 Notice of Continued Operation Internal distribution

   Not for issue. Circulated to a distribution list of nobody, and therefore never read, and therefore still accurate.

3. SN-03 No Such Document Issued automatically

   Issued on request for anything the Office does not hold, including itself. Following the link demonstrates the form more completely than reading about it.

4. SN-04 Unauthorized Sprint Naming Incident Report Not held

   Filed by teams who named a sprint without convening. No copy was retained, on the grounds that retention would constitute acknowledgement.

5. SN-05 Designation Dispute Form Not held

   Disputes are referred on receipt to the committee. Referrals to date: none. Disputes to date: several.

6. SN-06 [WITHDRAWN]

   Withdrawn on the advice of the committee that drafted it. Requests for access to restricted designations were made on this form.

7. SN-07 [ACCESS RESTRICTED]

   Access is granted by the committee, on application. Applications are made on Form SN-06.

8. SN-08 [DO NOT REPRODUCE]

   Held. The instruction appears on the document and nowhere else, which is why it appears here without it.
